Most home care websites make you book a call to hear a price. Here are the real numbers first: in 2026, agency home care in the GTA generally runs $28 to $45 per hour for PSW-level support, with Toronto at the top of that band. What moves the hourly rate
- Complexity of care. Companionship sits at the lower end of the range; dementia support and post-surgical care may cost more because they require additional training or clinical RN involvement.
- When care happens. Overnights, weekends and holidays price above weekday daytime.
- Visit length and minimums. Many agencies won’t book under 3–4 hours; short visits carry premium rates when offered at all.
- Structure. Hourly, overnight block, live-in and 24-hour rotations are priced differently — and the right structure often costs less than the obvious one.
A worked example
Say Mom in Mississauga needs mornings: two hours a day, seven days a week, for bathing, medications and breakfast. At the $35/hour starting rate, that’s $490 a week — roughly $2,100 a month. Compare that to GTA retirement homes commonly advertising $4,000 to $8,000 a month, and to giving up a salary to do it yourself, and the math of staying home starts looking different than most families expect.
Agency vs. hiring someone privately
A privately hired caregiver in Ontario typically asks $20 to $28 an hour — cheaper on paper. The difference is that you become the employer: payroll and source deductions, scheduling, vetting, and the 6 a.m. scramble when they’re sick. An agency’s rate carries the backup caregiver, the background checks, and in our case a dedicated care manager designing and reviewing the plan. Cheap gets expensive the first week something goes wrong.
When the price is $0
Some GTA families pay little or $0 out of pocket for approved care. If care follows a car accident, Ontario auto insurance may fund attendant care (billed through HCAI). After a workplace injury, WSIB may fund personal care. Eligible newcomers may qualify under IFHP. Publicly funded Ontario Health atHome visits can also run alongside private hours. Overnight and live-in: how the math changes
Hourly rates stop being the right lens once needs pass roughly 8 to 10 hours a day. Overnight support is usually priced as a shift block rather than stacked hours, and live-in or 24-hour care uses flat structures that come out well below 24 × the hourly rate. The practical move: ask for the quote both ways. We do that by default (hourly versus structured, side by side, in writing) because the “obvious” option is wrong surprisingly often.
The tax angle most GTA families miss
Attendant care expenses can qualify for the medical expense tax credit, and family members supporting a dependent may qualify for the Canada Caregiver Credit. We’re caregivers, not accountants, so treat this as a pointer rather than advice — but keep every invoice, and put one meeting with your accountant on the list. Families are routinely surprised by what a year of receipts recovers.
What publicly funded hours actually provide
Ontario Health atHome visits are assessment-based and task-focused: a bathing visit, a nursing visit, a block of PSW time. As a layer, excellent — and free. As the whole plan for heavier needs, rarely enough, which is why most of our families run public hours and private hours side by side. We schedule around the public visits so your paid hours land exactly where the public ones can’t reach: early mornings, evenings, nights, weekends.
How to compare quotes without getting burned
Ask any agency three things: Is the rate written down before care starts? What exactly triggers a higher rate? And who clinically owns the care plan? If the answers are vague, the invoice won’t be. Ours are on the pricing page, in public, which tells you how we feel about buried fees.
